Maternity

Comprehensive care (medicine, nursing, referrals for social services, nutrition, and health education) provided for maternity patients and their infants.

Eligibility

Confirmation of pregnancy required.  For quicker entrance to prenatal care call (386) 437-7350 ext. 2213 for pregnancy test to facilitate application for Temporary Medicaid good for 45 days.  Please bring Photo ID, Birth Certificate, Social Security Card and 1 months worth of income for working household members.  Income must be current and consecutive.
