Services include Well and Sick Child/Adult visits and Primary Care, Ages birth - 45 years of age.  Call Medicaid Options 1-888-367-6654 for enrollment into Medipass with the Flagler County Health Department.

School Physicals

School Physicals for school and daycare entrance are done by appointment.  The cost is $50.00.  To schedule an appointment call (386) 437-7350 ext. 2237.
