The Flagler County Health Department is a Vaccine for Children (VFC) vaccine provider. We give all shots necessary for Childcare centers and schools.  DOH Form 680 ("Blue Cards") are available upon request.  Religious Exemptions for immunizations (DOH Form 681) can be obtained through our Immunization Department nurse.  It is important that you and your family receive the proper vaccinations. The Immunization Center provides:

  • Adult and Children Vaccinations
  • Back to School Immunizations
  • Influenza Vaccines
  • Pneumonia Vaccines
  • Other Diseases and Infections Vaccines


Vaccines that are required for school and child care attendance are FREE for clients up to 18 years of age. For those clients 19 years and above there is a fee for vaccines.
