Primary Care
This is a local program that provides basic medical care for eligible children and adults on a sliding fee scale.  The emphasis of this program is on low income persons at or below 150% of the Federal Poverty Guidelines.  It does not cover prescriptions, dental services, referral to specialists, hospitalization, emergency room visits, X-Rays, CT, MRI, EKG, Ultrasounds and invasive or non-invasive testing.  For eligibility information call 386-437-7350 ext. 2230.

School Physicals 
School Physicals for school and daycare entrance are done by appointment.  The cost is $50.00.  To schedule an appointment call 386-437-7350 ext. 2237.
